• Define Defect Prevention Plan at organization as well as project level.

• Provide solutions to the technology related problem related to cycle time, productivity and rework Develop new tools/ methods .

• Identify defect classification schemes, common defects and common root causes using SPDB at the beginning of the project .

• Update defect database and monitor the occurrence of defects to take appropriate action and revise Defect Prevention Checklist.

• Conduct phase kick-off meeting and do causal analysis using Fishbone and Pareto Analysis at the end of the phase.

• Few DP results
  - Cost of Failure/cost of Production reduced by 22%
  - Cost of Failure/cost of Prevention reduced by 32%
  - Customer Satisfaction increased by 37%.
